  Facelift

  Facelifts have come a long way since the surgery was first performed in the early 1900’s. Facelift techniques have improved due to experience and a better understanding of the underlying anatomy and characteristics of the face. The primary goal of facelift surgery is to reposition skin, tighten underlying muscle and reduce excessive fat. As we age, skin often becomes loose, cheeks hollow and fat and wrinkles develop on the neck. Many patients start noticing aging symptoms (fine lines, wrinkles, sagging skin, etc) in their late 30’s, so most facelift candidates are in the 45-70 year old age range. However, an older patient who is in good health may also be an ideal candidate. Since everyone ages differently, each surgical case is determined on a case by case basis. During your consultation, I will assess the current state of your skin and tissues and develop a plan of action that best addresses your needs. Patients have several worries as they consider a facelift. They are concerned if their procedure will be safe and if they will have any complications. Your best assurance that your procedure will be smooth and without difficulties is to select a surgeon with extensive training and a good deal of experience with the facelift procedure. • Mini Facelift

  The reality is that it is somewhat artificial to strictly divide facelifts as either being “full facelifts” or “mini-facelifts.” Each individual presents with unique anatomic signs of aging. If those signs of aging are less advanced, then the facelift procedure I select will be adapted to address those changes with less extensive work. As an example, many patients have a reasonable amount of excess tissue under their chins. In these cases, a small incision (sub-mental incision) is made in order to allow tightening of this area. However, there are some patients who have very minimal signs of aging below their chins, in which case the sub-mental incision can be avoided and a terrific result obtained none-the-less. The point here is that each procedure is customized, and I will do what is necessary to achieve a quality result and no more. Many different surgical techniques and styles exist when it comes to both a “deep-plane” facelift and mini facelift surgery. I will determine which surgical approach is best for you based on your individual goals and where you currently are in the aging process.   Rhinoplasty

  Rhinoplasty is a very elegant and complicated surgery that can provide tremendous functional and cosmetic benefits to the patient. • Revision Rhinoplasty

  Unfortunately, due to the complexity of nasal surgery, I sometimes see patients that are not satisfied with the results of a Rhinoplasty performed by another surgeon. I understand that deciding to move forward with any surgical procedure can be a scary and difficult process so I feel sympathetic when a patient goes through the process and are not happy with the end result. Fortunately, I have years of experience performing Rhinoplasty revisions in order to correct surgeries that may have been “too aggressive” or “not aggressive” enough – leaving the patient feeling as though no improvements have been made. For this reason, it is crucial to have clear and effective communication with your surgeon so that he/she thoroughly understands what you are hoping to achieve and can provide you with realistic expectations and goals. It is critical to find a surgeon that focuses specifically on the face to ensure that you are receiving care from a doctor who has extensive experience with the anatomy and structure of the nose. Few surgeons are highly specialized and qualified to perform Revision Rhinoplasty.   Blepharoplasty

  Upper Blepharoplasty: The eyes are an important focal point in the face and significantly impact the appearance of the face. As we age, the skin in our upper eyelid loosens which can cause hooding over the eyes and the appearance of smaller eyes. During upper blepharoplasty surgery, I remove excess skin and/or fat to open up the eyes and restore their previous youthful and rejuvenated look. Some women really benefit from this procedure because they are once again able to apply eye shadow to their upper eyelid without it smearing or wiping off. However, both men and women with extra eyelid skin can be great candidates for upper eyelid surgery. Lower Blepharoplasty: One of the most common complaints I hear from patients is a tired and weary appearance from under eye bags. Puffiness and excessive under eyelid skin can be reduced with lower blepharoplasty surgery. During surgery, I meticulously remove extra skin or fat underneath the eye to restore the eyes to its more youthful and vigorous appearance. Often times, upper and lower blepharoplasty surgery may be performed together to further enhance the aesthetic result and really make the eyes stand out. The recovery time after blepharoplasty surgery is approximately two weeks, but well worth it to patients who have been wanting to improve the look of their eyes for years. • Revision Blepharoplasty

  With every cosmetic surgery, no matter how skilled the surgeon there is an element of risk. Unfortunately, after investing ample time and money, some patients find that their surgical expectations have not been met (for cosmetic or functional reasons) and that revision surgery is necessary. I often recommend that patients wait a full year after their initial surgery (when swelling and healing resolves) before rushing into another surgical procedure. Flaws or defects present after surgery may resolve on their own within a couple months. Also, the true surgical results are often not seen until six months or more down the line when inflammation has resolved and healing has occurred. Often times, problems that occur after blepharoplasty surgery are from too much or too little skin taken off. In either case, it is crucial to find a specialized surgeon with years of experience and expertise to repair the problem. Operating on previously incised skin and tissue (especially in a tender area such as around the eyes) can be more challenging and presents more risk and complications. • Cheek Lift (Mid Face Lift)

  One of the most prominent indications of aging is evident in the skin and often starts to occur in our early thirties. As we age, our skin and tissues slowly begin to droop and wrinkles and lines begin to form. Cheeks that were once full and high, begin to sag, lose contour and can appear hollow. A Cheek Lift (mid-face lift) can help restore cheeks to their once previous youthful appearance, reduce cheek folds and lower eyelid bags or hollowness. Often, a patient in need of a rejuvenation in the mid face also needs improvement in the lower face, jowl and neck area; therefore a face/neck lift might be the best treatment option. We will examine all of your options and concerns during a detailed consultation.   Forehead Lift (Brow Lift)

  We all want our face to accurately express how we are feeling. However as we age, our muscles and skin tend to droop, which can make us look grumpy, upset, angry or worn-out. The first signs of aging tend to occur in the crowsfeet and forehead area. The goal of forehead lift surgery (also known as browlift surgery) is to elevate the brow into its previous more youthful position and reduce deep forehead lines and creases. Please note, that the goal of a browlift is not to remove all the horizontal lines on the forehead. This often cannot be achieved with a browlift procedure. If you’re main goal is to address the horizontal lines on the forehead, Botox® Cosmetic may be a better approach. Browlift surgery is mostly designed to improve the position of the brow, not improve skin characteristics like wrinkles. Some women find it difficult to apply and keep makeup on the upper eyelids due to the hooding of the lids. In order to test if you might be good candidate for a forehead lift, lift the skin on top of your eyebrows into a higher position. If the excess skin in the eyebrow crease reduces, you may be a good candidate. A forehead lift effects the upper third of the face. If you want to address the mid face, you may find that a cheek lift or facelift is a better option for you. Often times, patients see the best results when combining more than one procedure to address all of their cosmetic concerns. Most patients who undergo a forehead lift are between the ages of 40-60 years old; however because of genetics, a forehead lift may be a reasonable for someone younger. • Facial Implants

  Facial implants can be used to contour, augment and enhance areas of the face lacking definition or character. With the right size and placement, facial implants can restore the proportion and symmetry of the face, while enhancing attractive features. Common areas where facial implants are often inserted include the chin (which can often blend in with the neck), cheeks (which can appear hollow) and jawline (which can lack definition). Chin implants are often performed in combination with a facelift or rhinoplasty surgery to address a small chin. Some of the most attractive people have pronounced jawlines, projected chins, and full, defined cheeks. It is rumored that blonde bombshell Marilyn Monroe had a chin implant in 1950, but this is highly debated due to her natural looking appearance. In addition to enhancing facial features, facial implants can be inserted with the goal of making other facial features appear less prominent (for example: augmenting a chin may make the nose appear smaller). Facial implants can also be implanted for a more youthful and rejuvenated look or to correct facial deformities formed at birth or from injury. • Facial Liposuction

  Excess fat in the face can actually make you appear heavier and make you look like you weigh more than you do. Even with moderate exercise and healthy eating, facial fat can be difficult to get rid of especially underneath the chin, on the neck, cheeks and jawline. Facial liposuction can be a great solution to reduce stubborn facial fat and remove the appearance of a “double chin,” which is often a heredity trait. It can also help achieve a more aesthetically pleasing facial contour by re-sculpting areas around the chin, neck, jowls or cheeks. Both younger and older patients may benefit from the long lasting results of facial liposuction. Many older patients see greater benefits when combined with another surgical procedure such as a face lift, neck lift, blepharoplasty or brow lift. Good candidates are often men and women with fatty deposits and sagging skin in the jowl area, lower face and chin. • Neck Lift/Liposuction

  The neck can be one of the first primary indicators of age. Many patients call our office with complaints of a saggy neck, “turkey waddle,” multiple chins, neck bands, and/or wrinkles. As we age, underlying neck muscles begin to relax and skin begins to lose its elasticity leading to neck bands, “turkey neck” and the appearance of “double chins.” Symptoms can further be exacerbated by poor genetics, stress and weight fluctuations. Most patients with aging neck concerns range from 40-70 years old. The purpose of Neck Lift surgery (cervicoplasty) is to reduce loose neck skin and excess fat, smooth out wrinkles/bands and firm neck muscles and skin. The result is a tighter, more youthful looking neck with greater definition and contour. The best candidate for a Neck Lifting procedure is a healthy man or woman with excessive neck skin or fat.   Fat Transfer Injections

  Many people today are trying hard to lose fat and maintain a healthy weight. Of course, keeping your weight appropriate is a wonderful goal that helps you achieve your aesthetic goals and will also pay big dividends for cardiac health. However, from the standpoint of facial aesthetics, there is a tough tradeoff. As our face ages there are three things that happen simultaneously: facial tissues sag (which can be addressed with a facelift), the skin develops wrinkles and age spots (which can be addressed with Botox® Cosmetic, laser skin treatments, etc), and the face slowly losses volume and becomes somewhat hollow appearing. It is this last factor of facial devolumization that I am discussing on this page. What can fat transfer injections achieve? There are some instances in which fat transferred into areas of the face can drastically improve the overall facial appearance and lead to a more youthful and rejuvenated look. When we lose weight in the face, it can appear hollow and aged. To make matters worse, as we age tissues descend and the skin thins out and forms a variety of lines and wrinkles (due to fat loss, facial expressions made throughout the years, sun exposure, smoking, genetics, etc.). Fat transfer injections (also known as microlipoinjections) restore lost volume and sculpt your face into its previous youthful and vibrant self. Fat injections are non-invasive (meaning they do not require a linear incision) and can benefit those looking to smooth out wrinkles, lines and folds in the skin.   Earlobe Repair

  Earlobe surgery is a simple procedure done in the office here at Sistine Facial Plastic & Laser Surgery to repair torn or enlarged earlobes. Many patients want to repair the tear or slit caused by years of wearing heavy or dangling earrings. In Pittsburgh and other parts of the country, ear repairs to address gauged ear holes have become more common.   Belotero Balance®

  The key difference between Belotero® and other dermal fillers is that it can be injected very superficially. BeloteroBelotero® is an exceptionally soft and smooth facial filler and therefore is very unlikely to cause bumps or lumps even when injected very superficially for the purpose of addressing fine lines. Because it is possible to inject Belotero® Balance so superficially it works in a similar way as collagen injections were used years ago – to efface or smooth lines and wrinkles. Another advantage of Belotero® is that it has been designed to avoid the Tyndall effect. The Tyndall effect is the appearance of a faint blue color under the skin when other cosmetic fillers are injected just under the skin. Belotero® does not cause this problem. BELOTERO_PackageBelotero® is an innovative dermal filler used to provide a temporary solution for bothersome sigs of aging.   I believe that Dysport® has been a very useful addition to the options available to patients to combat the slow accumulation of lines and wrinkles on the face. Dysport® joins another excellent neurotoxin, BOTOX® Cosmetic, as a powerful tool to treat, and to a certain degree prevent, the appearance of wrinkles in the first place. What is Dysport®? Dysport® is a muscle paralyzer that temporarily evens out wrinkles and creases to create softer, younger-looking skin. Every time you make a facial expression (such as laughing, smiling or squinting) different areas of your face deepen into folds and creases. As we get older and lose our skin elasticity, those lines and wrinkles become more pronounced. Dysport® is often injected into many of the same areas where Botox® Cosmetic is used, such as the forehead, crows’ feet, lines between eyebrows, frown lines, lines running alongside of nose, wrinkles above the lip and creases around the neck.   Juvederm®

  JUVÉDERM® is a facial filler that temporarily softens wrinkles, creases and folds by filling in areas with volume loss. It is composed of hyaluronic acid – a substance that plays a key part in the aging process and has revolutionized the world of fillers. As we age, we lose hyaluronic acid and collagen begins to break down. Injectable fillers like JUVÉDERM®, Restylane® and Perlane® start working immediately to replenish and volumize areas that have lost support and structure due to the aging process. JUVÉDERM® is often injected into wrinkles, creases, folds and other sunken areas of the face to rejuvenate and revitalize the skin. Treatment only takes approximately 15 minutes, but the effects can last 6 months or longer. I inject most of the various fillers available in the marketplace. Our experience is that certain fillers work slightly better in specific areas of the face than others. Furthermore, some patients have a strong preference for a specific filler because they obtained good results with it in the past. For both of these reasons, we like to provide our patients with choices. • Kybella™

  One of the most stubborn areas to remove or reduce fat is in the chin area. Since weight loss is not very effective in diminishing a “double chin,” liposuction has been a common procedure to treat this condition. However, liposuction is a surgical procedure requiring a visit to the hospital and approximately two weeks of downtime. shutterstock_121013587Kybella offers a non-surgical alternative to treating chin fat without a long recovery period or a trip to the hospital. With a series of injections, Kybella breaks down fat cells which are then reabsorbed into the body. The chemical component of Kybella is very similar to a naturally occurring substance in the body called deoxycholic acid.   Microcannulas

  The introduction of microcannulas has significantly improved the technique used to inject facial fillers. A microcannula is a small, flexible, blunt-tip needle that can be used to inject a variety of fillers including Restylane®, Perlane®, Beletero® Balance, JUVÉDERM®, and Sculptra Aesthetic®. Cosmetic filler injections have become a very popular treatment for patients in Pittsburgh and around the country. The single most significant concern for patients who have filler injections is the social downtime as a result of possible bruising from the injections. The use of microcannulas very significantly reduces the chance of bruising with these treatments. Microcannulas have largely replaced the use of sharp needles in Europe and are rapidly gaining popularity in the United States.   Dr. Paul Leong prides himself on keeping up with the latest advancements in the cosmetic industry and is proud to announce the availability of Restylane® Silk – a new FDA approved dermal filler.   Sculptra®

  I feel that Sculptra® Aesthetic is an outstanding treatment to restore volume to the thinning face. As we age, there are fundamentally two things that happen: the soft tissue of the face descends, and we slowly lose facial volume. The decent of facial soft tissue can be addressed surgically (with a facelift) or non-surgically (with Ultherapy®). If a patient is going to the operating room, I often choose to perform fat transfer to restore facial volume. However, if the patient prefers not to go to the OR, Sculptra® Aesthetic can be a terrific non-surgical option. Sculptra® Aesthetic (sometimes known as a Liquid Face Lift) is a longer-lasting anti-aging facial injectable used for loss of facial volume, particularly the “flatter” areas of the face such as the cheeks and the temple areas. By contrast, for the natural “grooves” of the face, such as the nasolabial folds, tear troughs, and marionette lines, I prefer to use a hyaluronic acid injectable such as Perlane®, Restylane®, or JUVÉDERM®. • Acne Scarring

  As if struggling with acne is not uncomfortable enough, many patients both young and old are left with scarring as a constant reminder of their battle with this skin disease. As many as 80% of people will have acne by the time they reach their teens or twenties. Acne scarring can make you feel self-conscious, insecure and embarrassed. Fortunately, there is help to diminish the negative effects of acne scarring. Many patients are initially interested in a way to cure acne scars by using a topical treatment. They have often searched for the best cream for acne scars, home remedies for acne scars, or perhaps tried microdermabrasion for acne scars. Frequently, I find that patients have been disappointed in these treatments. In order to have effective acne scar treatment, resurfacing of the skin with an optimal laser resurfacing procedure is often the best approach. It is important to understand that treating laser scars is distinct from treating active acne. Treating active acne is best addressed by a medical doctor or dermatologist.   If you are seeking a quick and easy solution to sagging brows without having surgery, a BOTOX® Browlift might be a great option for you.   I find it’s frequently the case that as people age they tend to focus on their face and neglect an area that can easily reflect signs of aging and maturity… their hands. Our hands are in constant view as we communicate and express ourselves. Over the years, hands begin to develop age spots, loose volume, and veins become more prominent. Restoring youthful hands is all about improving texture and replacing lost volume. Thick skin, pigmentation patches, brown and red spots, unsightly veins and tendons, a “bony” appearance and fine lines – all become prominent in aging hands and can occur even in our forties. Much like our faces, our hands show signs of aging from unavoidable sun exposure, loss of fat, collagen, elasticity and heredity.   Latisse® Eyelash Enhancer

  The eyes are often one of the first features that people notice so why not make them look their very best? Latisse® is an FDA approved prescription product that darkens, thickens and lengthens eye lashes. It can be a great option for those at least 18 years of age, in good health, with poor eye lashes and the desire for long, luscious lashes! An ingredient in prescription eye drops for people with glaucoma was discovered as an agent that grows eyelashes naturally. The makers of the Lumigan eye drops (Allergan) decided to separate this component responsible for eyelash growth (Bimatoprost) and manufacture it for cosmetic use for people with Hypertrichosis – short, thinning lashes. It is believed that Latisse® affects the growth phase of the hair by increasing length and number of hairs. By 16 weeks participants in a research study revealed that eyelash length improved by 25 percent, thickness and fullness improved by 106 percent, and lashes were 18 percent darker. • Lip Augmentation

  Full lips continue to be a sign of youth and beauty. As we age and our hormonal balance changes, lips become thinner, loose definition and begin to sag. Hyaluronic Fillers such as Restylane®, Juvederm® Ultra, Juvederm® Ultra Plus, and Perlane® provide an excellent non-surgical solution to shape and volumize thin, aging lips. More than a decade ago plastic surgeons often used collagen lip injections for augmentation. However, we found that collagen for lips tends to last a very short time, so today most plastic surgeons prefer the hyaluronic acid fillers. Lip fat transfer provides a more permanent option for full, voluptuous lips. When it comes to lip augmentation, my goal is to create full, soft, natural looking lips that are in proportion with the face. I certainly see some patients with bad lip injections come into my office. The most common issue with these patients is that too much filler has been injected, leading to excessive and artificial appearing lips. In order to make sure that the patient and I have a similar goal for how their lips will look, I often review a series of lip augmentation before and after pictures.   Non-Surgical Rhinoplasty

  Does the appearance of your nose bother you, but the thought of surgery scare you? If so, a Non-Surgical Rhinoplasty could be a great option for you. A Non-Surgical Rhinoplasty is a fairly new, innovative technique that can smooth out a hump in the nose, improve symmetry, and reduce nasal deformity using hyaluronic acid fillers. One of the great benefits of using a filler (such as Restylane®, Perlane® or Juvederm®) to correct the nose is the ability to see immediate results and get an idea of what your nose could look like if you decide to move forward with surgery. I perform all of the Non-Surgical Rhinoplasty injections in our Pittsburgh, Sewickley, Wexford and Greensburg offices and have extensive experience reshaping the nose, so you can be assured that you are receiving quality, specialized service. I will decide if you are a good candidate for a Non-Surgical Rhinoplasty during your consultation.   Tear Trough Correction

  Tear trough deformity (sometimes referred to as: dark circles under eyes, under-eye hollows, or hereditary dark circles) is a very common cosmetic complaint. Happily there is a fairly effective non-surgical approach to improving this issue. When the area under the eyes is dark, hollow, or drooping it can substantially impact the appearance of the face. Under-eye hollowness can be due to a number of different factors, including genetics, allergies, or aging. In order to correct tear trough deformity, I inject hyaluronic acid fillers such as Restylane® underneath the eyes to fill in hollowness and reduce darkness. Tear trough correction can offer a wonderful non-surgical solution to a tired, weary and aged appearance by restoring lost volume, getting rid of the under-eye dark circles, and rejuvenating the tear trough area. Who is a good candidate for Tear Trough Correction or eye treatment for dark circles? Any man or woman over the age of 18 years old who has darkness, hollowness or drooping under the eyes or in the cheeks can be a good candidate for Tear Trough Correction.

  Stretch Marks

  appreciate that problematic stretch marks can be a bothersome and emotional topic for many patients. Many people will gain and lose weight in their lifetime, which can lead to accelerated stretching of the skin and unsightly stretch marks (also referred to as striae). Stretch marks occur when your body mass accelerates faster than your skin. Eventually elastic fibers in the skin begin to pull and break when stretched to the limit. Stretch marks are especially common after pregnancies (pregnancy marks) due to rapid weight gain and hormonal fluctuations (approximately 90% of women will get stretch marks in the last few months of their pregnancy). Hormonal imbalance can cause tearing in the outer layer of skin called the epidermis. Heredity, diet, ethnicity and exercise also play a role in the formation of stretch marks. Both men and women can get stretch marks, however it is more commonly seen in women, most likely due to higher level of hormonal fluctuations. Common areas of the body prone to stretch marks include, but are not limited to: the arms, thighs, breasts, hips, lower back, stomach and buttocks. Unfortunately there is no way to prevent stretch marks, however maintaining a good skin care regime and keeping your skin well hydrated can help.
