Facelifts have come a long way since the surgery was first performed in the early 1900’s. Facelift techniques have improved due to experience and a better understanding of the underlying anatomy and characteristics of the face. The primary goal of facelift surgery is to reposition skin, tighten underlying muscle and reduce excessive fat. As we age, skin often becomes loose, cheeks hollow and fat and wrinkles develop on the neck. Many patients start noticing aging symptoms (fine lines, wrinkles, sagging skin, etc) in their late 30’s, so most facelift candidates are in the 45-70 year old age range. However, an older patient who is in good health may also be an ideal candidate. Since everyone ages differently, each surgical case is determined on a case by case basis. The reality is that it is somewhat artificial to strictly divide facelifts as either being “full facelifts” or “mini-facelifts.” Each individual presents with unique anatomic signs of aging. If those signs of aging are less advanced, then the facelift procedure I select will be adapted to address those changes with less extensive work. As an example, many patients have a reasonable amount of excess tissue under their chins. In these cases, a small incision (sub-mental incision) is made in order to allow tightening of this area. However, there are some patients who have very minimal signs of aging below their chins, in which case the sub-mental incision can be avoided and a terrific result obtained none-the-less. The point here is that each procedure is customized, and I will do what is necessary to achieve a quality result and no more. Many different surgical techniques and styles exist when it comes to both a “deep-plane” facelift and mini facelift surgery. Unfortunately, due to the complexity of nasal surgery, I sometimes see patients that are not satisfied with the results of a Rhinoplasty performed by another surgeon. Upper Blepharoplasty: The eyes are an important focal point in the face and significantly impact the appearance of the face. As we age, the skin in our upper eyelid loosens which can cause hooding over the eyes and the appearance of smaller eyes. During upper blepharoplasty surgery, I remove excess skin and/or fat to open up the eyes and restore their previous youthful and rejuvenated look. Some women really benefit from this procedure because they are once again able to apply eye shadow to their upper eyelid without it smearing or wiping off. However, both men and women with extra eyelid skin can be great candidates for upper eyelid surgery. Lower Blepharoplasty: One of the most common complaints I hear from patients is a tired and weary appearance from under eye bags. Puffiness and excessive under eyelid skin can be reduced with lower blepharoplasty surgery. During surgery, I meticulously remove extra skin or fat underneath the eye to restore the eyes to its more youthful and vigorous appearance. With every cosmetic surgery, no matter how skilled the surgeon there is an element of risk. Unfortunately, after investing ample time and money, some patients find that their surgical expectations have not been met (for cosmetic or functional reasons) and that revision surgery is necessary. I often recommend that patients wait a full year after their initial surgery (when swelling and healing resolves) before rushing into another surgical procedure. Flaws or defects present after surgery may resolve on their own within a couple months. Also, the true surgical results are often not seen until six months or more down the line when inflammation has resolved and healing has occurred. Often times, problems that occur after blepharoplasty surgery are from too much or too little skin taken off. We all want our face to accurately express how we are feeling. However as we age, our muscles and skin tend to droop, which can make us look grumpy, upset, angry or worn-out. The first signs of aging tend to occur in the crowsfeet and forehead area. The goal of forehead lift surgery (also known as browlift surgery) is to elevate the brow into its previous more youthful position and reduce deep forehead lines and creases. Please note, that the goal of a browlift is not to remove all the horizontal lines on the forehead. This often cannot be achieved with a browlift procedure. If you’re main goal is to address the horizontal lines on the forehead, Botox® Cosmetic may be a better approach. Browlift surgery is mostly designed to improve the position of the brow, not improve skin characteristics like wrinkles. Some women find it difficult to apply and keep makeup on the upper eyelids due to the hooding of the lids. In order to test if you might be good candidate for a forehead lift, lift the skin on top of your eyebrows into a higher position. If the excess skin in the eyebrow crease reduces, you may be a good candidate. A forehead lift effects the upper third of the face. Facial implants can be used to contour, augment and enhance areas of the face lacking definition or character. With the right size and placement, facial implants can restore the proportion and symmetry of the face, while enhancing attractive features. Common areas where facial implants are often inserted include the chin (which can often blend in with the neck), cheeks (which can appear hollow) and jawline (which can lack definition). Excess fat in the face can actually make you appear heavier and make you look like you weigh more than you do. Even with moderate exercise and healthy eating, facial fat can be difficult to get rid of especially underneath the chin, on the neck, cheeks and jawline. The neck can be one of the first primary indicators of age. Many patients call our office with complaints of a saggy neck, “turkey waddle,” multiple chins, neck bands, and/or wrinkles. As we age, underlying neck muscles begin to relax and skin begins to lose its elasticity leading to neck bands, “turkey neck” and the appearance of “double chins.” Symptoms can further be exacerbated by poor genetics, stress and weight fluctuations. Most patients with aging neck concerns range from 40-70 years old. Many people today are trying hard to lose fat and maintain a healthy weight. Of course, keeping your weight appropriate is a wonderful goal that helps you achieve your aesthetic goals and will also pay big dividends for cardiac health. However, from the standpoint of facial aesthetics, there is a tough tradeoff. The key difference between Belotero® and other dermal fillers is that it can be injected very superficially. I believe that Dysport® has been a very useful addition to the options available to patients to combat the slow accumulation of lines and wrinkles on the face. One of the most stubborn areas to remove or reduce fat is in the chin area. The introduction of microcannulas has significantly improved the technique used to inject facial fillers. A microcannula is a small, flexible, blunt-tip needle that can be used to inject a variety of fillers including Restylane®, Perlane®, Beletero® Balance, JUVÉDERM®, and Sculptra Aesthetic®. Cosmetic filler injections have become a very popular treatment for patients in Pittsburgh and around the country. The single most significant concern for patients who have filler injections is the social downtime as a result of possible bruising from the injections. The use of microcannulas very significantly reduces the chance of bruising with these treatments. I feel that Sculptra® Aesthetic is an outstanding treatment to restore volume to the thinning face. As we age, there are fundamentally two things that happen: the soft tissue of the face descends, and we slowly lose facial volume. The decent of facial soft tissue can be addressed surgically (with a facelift) or non-surgically (with Ultherapy®). If a patient is going to the operating room, I often choose to perform fat transfer to restore facial volume. However, if the patient prefers not to go to the OR, Sculptra® Aesthetic can be a terrific non-surgical option. Sculptra® Aesthetic (sometimes known as a Liquid Face Lift) is a longer-lasting anti-aging facial injectable used for loss of facial volume, particularly the “flatter” areas of the face such as the cheeks and the temple areas. By contrast, for the natural “grooves” of the face, such as the nasolabial folds, tear troughs, and marionette lines, I prefer to use a hyaluronic acid injectable such as Perlane®, Restylane®, or JUVÉDERM®. Other than the formation of wrinkles and lines, one of the most prevalent signs of aging is the appearance of age or liver spots and vessels. As if struggling with acne is not uncomfortable enough, many patients both young and old are left with scarring as a constant reminder of their battle with this skin disease. As many as 80% of people will have acne by the time they reach their teens or twenties. Acne scarring can make you feel self-conscious, insecure and embarrassed. Fortunately, there is help to diminish the negative effects of acne scarring. Many patients are initially interested in a way to cure acne scars by using a topical treatment. I find it’s frequently the case that as people age they tend to focus on their face and neglect an area that can easily reflect signs of aging and maturity… their hands. Our hands are in constant view as we communicate and express ourselves. Over the years, hands begin to develop age spots, loose volume, and veins become more prominent. Restoring youthful hands is all about improving texture and replacing lost volume. The eyes are often one of the first features that people notice so why not make them look their very best? Latisse® is an FDA approved prescription product that darkens, thickens and lengthens eye lashes. It can be a great option for those at least 18 years of age, in good health, with poor eye lashes and the desire for long, luscious lashes! An ingredient in prescription eye drops for people with glaucoma was discovered as an agent that grows eyelashes naturally. The makers of the Lumigan eye drops (Allergan) decided to separate this component responsible for eyelash growth (Bimatoprost) and manufacture it for cosmetic use for people with Hypertrichosis – short, thinning lashes. It is believed that Latisse® affects the growth phase of the hair by increasing length and number of hairs. By 16 weeks participants in a research study revealed that eyelash length improved by 25 percent, thickness and fullness improved by 106 percent, and lashes were 18 percent darker. Full lips continue to be a sign of youth and beauty. As we age and our hormonal balance changes, lips become thinner, loose definition and begin to sag. Hyaluronic Fillers such as Restylane®, Juvederm® Ultra, Juvederm® Ultra Plus, and Perlane® provide an excellent non-surgical solution to shape and volumize thin, aging lips. More than a decade ago plastic surgeons often used collagen lip injections for augmentation. However, we found that collagen for lips tends to last a very short time, so today most plastic surgeons prefer the hyaluronic acid fillers. Does the appearance of your nose bother you, but the thought of surgery scare you? If so, a Non-Surgical Rhinoplasty could be a great option for you. A Non-Surgical Rhinoplasty is a fairly new, innovative technique that can smooth out a hump in the nose, improve symmetry, and reduce nasal deformity using hyaluronic acid fillers. Tear trough deformity (sometimes referred to as: dark circles under eyes, under-eye hollows, or hereditary dark circles) is a very common cosmetic complaint. Happily there is a fairly effective non-surgical approach to improving this issue. When the area under the eyes is dark, hollow, or drooping it can substantially impact the appearance of the face. Under-eye hollowness can be due to a number of different factors, including genetics, allergies, or aging. In order to correct tear trough deformity, I inject hyaluronic acid fillers such as Restylane® underneath the eyes to fill in hollowness and reduce darkness. Tear trough correction can offer a wonderful non-surgical solution to a tired, weary and aged appearance by restoring lost volume, getting rid of the under-eye dark circles, and rejuvenating the tear trough area. appreciate that problematic stretch marks can be a bothersome and emotional topic for many patients. Many people will gain and lose weight in their lifetime, which can lead to accelerated stretching of the skin and unsightly stretch marks (also referred to as striae). Stretch marks occur when your body mass accelerates faster than your skin. Eventually elastic fibers in the skin begin to pull and break when stretched to the limit. Stretch marks are especially common after pregnancies (pregnancy marks) due to rapid weight gain and hormonal fluctuations (approximately 90% of women will get stretch marks in the last few months of their pregnancy). Hormonal imbalance can cause tearing in the outer layer of skin called the epidermis. Heredity, diet, ethnicity and exercise also play a role in the formation of stretch marks. Both men and women can get stretch marks, however it is more commonly seen in women, most likely due to higher level of hormonal fluctuations. Common areas of the body prone to stretch marks include, but are not limited to: the arms, thighs, breasts, hips, lower back, stomach and buttocks. Unfortunately there is no way to prevent stretch marks, however maintaining a good skin care regime and keeping your skin well hydrated can help.
